[cgiapp] problem when combining CGI::Application::Dispatch and CGI.pm's url() and self_url() method

I believe there's a bug in CGI.pm which affects CGI::Application::Dispatch
users who use the mod_rewrite recipe recommended in the documentation.

I've filed a bug with CGI.pm which explains issue and supplies one proposed
patch (which is not backwards compatibile), and describes another solution
would would be backwards compatibile):
